Sauk Prairie Healthcare Foundation is committed not only to the health of our patients, but to the health of the communities we serve.
Healthy Life Grants fund a wide variety of wellness projects, including hiking and biking trails, literacy programs, senior fitness equipment, CPR training mannequins, defibrillator purchases, EMT and lifeguard training, school and community gardens, and local food pantries.
Last year, your gifts helped us award more than $53,000 in grants to organizations within those communities. Since 2004, Sauk Prairie Healthcare Foundation has awarded more than $405,000 in grants.

How can you encourage young people to read over the summer? The Friends of the Lodi Public Library had a great idea to help enroll people in their summer reading program. With a grant from Sauk Prairie Healthcare Foundation, they hosted a Summer Library Program Kickoff Carnival last May.
The event included a climbing wall, an inflatable hoops challenge, a face painting station, and a caricaturist. This event brought in 1,000 participants. It also helped the Friends sign up more than 170 people for the Summer Library Program in just four hours. They ended up with more than 700 registrations for the summer.
In a letter of thanks to the Foundation, the Friends team said, “This wouldn’t have been possible without your generosity. Thank you for helping our library and our broader community!”
Organizations can apply for Healthy Life Grants starting in April. Sauk Prairie Healthcare Foundation’s Board of Directors Grants and Gifts Committee reviews the applications in October. Winners are announced in November for the following year.
Grant proposals for 2025 funding are being accepted through September 30, 2025.
