Community Health

Sauk Prairie Healthcare is committed to improving the health of the communities we serve. Over the next few years, we will collaborate with other healthcare organizations, municipalities, emergency service providers, schools, businesses, community groups and others to address our top community health priorities. These include:

  • Mental Health — access to services, training and education, reducing stigma
  • Substance Misuse — use and abuse of alcohol, drugs, nicotine
  • Healthy Lifestyles — eat smarter and move more to prevent chronic medical conditions
  • Social Isolation — caused by the lack of family and friends or geographic inaccessibility
  • Housing — that is safe and people can afford

Community health improvement efforts in the past include education, such as classes about childbirth, breastfeeding, CPR, diabetes prevention, nutrition, mental health first aid and weight management. We also provide exercise opportunities, grants to community groups, Mobile Meals, health fairs, prescription drug collection, support groups, and free lab and X-ray services for patients of the Good Neighbor Clinic.

We are pleased to present our Community Health Needs Assessment (CHNA) and our Community Health Improvement Implementation Plan (CHIIP). Our CHNA provides more detail about our community health priorities described above.
