Patient and Family Advisory Council

We value the voices of those we serve and actively seek feedback to guide improvements. We invite our patients or a family member to apply for membership to our Patient and Family Advisory Council. This group brings together individuals with firsthand experience of care and healthcare professionals to promote patient-centered care, enhance the patient experience, and improve the quality and safety of healthcare services.

What Is the Role of Council Members?

As a member of the Patient and Family Advisory Council, you will have the opportunity to:

  • Provide feedback and insight from the patient and family perspective.
  • Collaborate on initiatives to improve communication, service, and care delivery.
  • Review educational and informational materials for clarity and relevance.
  • Serve as ambassadors for Sauk Prairie Healthcare within the community.

How Does the Patient and Family Advisory Council Work?

The Council will consist of 8–12 members representing a diverse cross-section of Sauk Prairie Healthcare’s patient and family population. Members of the Council will meet for about 90 minutes every other month, with each member serving a two-year term. In addition:

  • Membership is voluntary and non-compensated.
  • Attendance at scheduled meetings is expected; missing more than two consecutive meetings may result in review of continued membership.

How Are Participants Selected?

If you are interested in serving on the Patient and Family Advisory Council, fill out the application form below. We will respond to your submission within five to seven working days to discuss your application further.

To ensure the Patient and Family Advisory Council reflects a diverse range of perspectives, you will be asked to participate in an interview as part of the application process. This conversation helps us learn more about your experiences and confirm that the council is a good mutual fit. Our goal is to create a collaborative group that represents the voices of patients and families in meaningful ways.
