What is MyHealth?

MyHealth is an Internet based service that provides patients and/or their proxies access to personal medical information and clinic services on-line. MyHealth is available through a secure internet connection at www.saukprairiehealthcare.org/MyHealth. This is offered as a convenience and courtesy by Sauk Prairie Healthcare to our patients and enrollment is voluntary.

How do I sign up?

Sauk Prairie Healthcare patients age 18 and older may request an account.

Sign up steps:

  1. Sign up during your next appointment. Ask a Registration Specialist for an enrollment request form.
  2. Activate an account. Complete your enrollment by clicking on the Activate my account link and entering the required information.
Is there a fee to use MyHealth?

Sauk Prairie Healthcare MyHeath is a free service.

Why do I need to supply an e-mail address?

Sauk Prairie Healthcare requires that you provide a current e-mail address to activate your account. A brief message will be sent to this e-mail address to alert you when new information has been posted to your MyHealth account, or to any other account to which you have access. No personal medical information is sent within these e-mail alerts. You will receive an alert when a new test result, or appointment reminder has been posted to your Sauk Prairie Healthcare MyHealth account.

Who do I contact if I have further questions?

If you have questions regarding information contained in your medical record, please contact your clinic. They will assist you directly or connect you to someone who can help.

Your Medical Records

What medical information is available in MyHealth?

MyHealth provides access to portions of your medical information stored in Sauk Prairie Healthcare’s medical record. This includes things such as your allergies, immunizations, prescriptions, laboratory results, diagnoses, appointments, and demographics.

MyHealth does not display all information from your medical record, nor does it give access to every clinic service. If you need information or services beyond what is available to you on-line, please call your clinic. If you need a complete copy of your medical record, please contact Sauk Prairie Healthcare for information about the Release of Information process.

When can I see my test results in MyHealth?

MyHealth displays lab results for tests collected at Sauk Prairie Healthcare locations. Most results are available after your physician has reviewed them, or automatically within 72 hours. Some results, such as genetic tests and tumor markers, are released at 14 or 30 days. Radiology results and other tests that are not available in MyHealth will be communicated to you by telephone or U.S. Mail. If you have any questions or concerns about a test result, please contact your provider.

If some of my health information on MyHealth is not correct, what should I do?

Your MyHealth information comes directly from your Sauk Prairie Healthcare electronic medical record. When viewing information within MyHealth you may notice information that is missing or incorrect. If this should occur, please report this to clinical staff or your provider at your next office visit. Your information will be corrected in your medical record at that time, and the update will appear in MyHealth.

Where did the medication list come from that is in MyHealth?

It is important to share a list of all medications including vitamins, herbals and over-the-counter items you are taking with all of your healthcare providers.

The list included in MyHealth is compiled from the most recent information entered at Sauk Prairie Hospital or reviewed during an office visit at one of our primary care clinics (River Valley Clinic, Wisconsin Heights Clinic, Lodi Clinic or Plain Clinic).

Multiple resources are used to keep your list as accurate as possible, but if you do find an error in the medication list please notify your provider at your next appointment.

MyHealth for My Family or Others I Care for

How can I access the MyHealth account of a child, spouse, parent, or other person I care for?

To request access to another person's MyHealth account, you must first activate a MyHealth account for yourself. Once your account is activated, log into your personal account and complete the appropriate form(s) from the "Other Accounts" menu. Access to other accounts is strictly controlled, and only granted with appropriate authorization and consent forms on file. All requests are reviewed by the Sauk Prairie Healthcare Medical Records department, and are normally processed in 1-3 business days from the date of receipt. Request a personal account here.

What information is available when accessing another person's account?

When accessing accounts for spouse, parent, child or other person, all standard features are available. This includes:

  • Test Results
  • Medications - View current medications
  • Allergies - View current allergies
  • Immunizations - View all immunizations
  • Health Issues – View current health issues when shared by the patient's provider
  • View future and past appointments scheduled at Sauk Prairie Healthcare
  • Demographics - View and update

After I Have Enrolled

I forgot my password. What should I do?

You may click the "Forgot password" link on the sign-in page to retrieve your password. If you need further assistance, please call Sauk Prairie Healthcare at 608-644-3044.

Where can I update my personal information (e.g., home address, e-mail or change my password)?

Log into MyHealth and select Profile and make the changes desired. You may also indicate if this change should apply to other members of your household.

Technical Questions

How is MyHealth secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal ID's, passwords, and medical record numbers. Each person controls their password, and the account cannot be accessed without that password.

I was logged out of MyHealth what happened?

We aim to protect your privacy and security of your information. While logged into MyHealth, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out. We recommend that you log out of your MyHealth session if you need to leave your computer for even a short period of time.

My temporary username and password did not work, what should I do?

For your security, your temporary username and password expire after 7 days and is only valid for one-time use. At the time you activate your account, you create an ID and Password that only you will know. If you have trouble logging in, please call Sauk Prairie Healthcare at 608-644-3044.
