Plain Clinic to Open at New Location This Fall

Sauk Prairie Healthcare (SPH) is pleased to announce that Plain Clinic will open at its new location at 1370 Cherry Street in Plain on October 5, 2026.

For generations, Plain Clinic has provided trusted healthcare close to home. The new location builds on that legacy with a thoughtfully renovated space designed to better meet the needs of patients, providers, and the community for years to come. The project marks a significant investment in the health and well-being of Plain and the surrounding communities.

The clinic will continue to provide the trusted primary care services patients have long relied on from Family Medicine physician Dr. Jeffrey Collins and nurse practitioner Natalie Hanson. The new facility will also introduce a new service for the Plain community: outpatient behavioral health counseling for adults, including individual psychotherapy provided by a licensed clinician.

"Our team has been honored to care for patients and families in Plain over the years, and we're excited to welcome them into a space that was designed specifically with their needs in mind," said Dr. Jeffrey Collins. "The new clinic will allow us to provide the same personalized care our patients know and trust in an environment that is more comfortable, accessible and better equipped to support the health of our thriving community."

The new clinic has been created through the renovation of the former River Valley School District Early Learning Center, giving new purpose to a familiar community building. Sauk Prairie Healthcare chose to renovate the existing structure as part of its commitment to the health, strength, and future of Plain and neighboring communities.

Designed with patients and caregivers in mind, the facility features larger exam rooms that provide greater flexibility for patient scheduling and care delivery. Patients will benefit from convenient parking and easy access to the clinic entrance. The fully renovated space has been designed to be both healing and comforting, creating a welcoming environment for patients and families.

Kraemer Brothers serves as the project's general contractor, and HGA is the project's architect. Following the clinic's opening, Sauk Prairie Healthcare will evaluate future opportunities for other portions of the building, including the gymnasium, kitchen, and classrooms, with potential uses focused on wellness programming and community health initiatives.

The Plain Clinic project is part of Sauk Prairie Healthcare Foundation's "Created Together. Growing Together." capital campaign, a transformative effort that builds on the community investment made in local healthcare over the past decade. More than $3 million has already been raised toward the campaign's $4.1 million goal, demonstrating strong support for projects that enhance local access to care and help keep healthcare independent and community-focused.

"Partnerships like these are essential to keeping healthcare independent, local, and responsive to the needs of the people we serve. As we continue our 'Created Together. Growing Together.' capital campaign, we will be asking the public to support this investment in Plain — an investment that will strengthen access to care and create lasting benefits for the entire community."

Additional details regarding a community open house and clinic transition plans will be announced closer to the October opening date.
