- Established patients of Sauk Prairie Healthcare primary care (Lodi Clinic, Plain Clinic, River Valley Clinic, or Wisconsin Heights Clinic) — to schedule an appointment, call your primary care clinic so they are aware of your needs and can assess the best care option for you
- Employees of Sauk Prairie Healthcare and their family members who reside in the same home but are not established patients — to schedule an appointment, call 608-644-3000
