Attention Seniors with Anthem Medicare Advantage Plans!
Effective April 1, 2024, Anthem Blue Cross Blue Shield may not allow Sauk Prairie Healthcare to participate as an in-network provider for their health insurance plans if a fair agreement cannot be reached in our contract negotiations.
If you wish to continue receiving care at Sauk Prairie Healthcare, you can switch from your current Anthem/Blue Cross Blue Shield Medicare Advantage plan to another plan before open enrollment ends on March 31, 2024.

Elder Benefit Specialist Pam Fuchs from the Sauk County Aging and Disability Resource Center (ADRC) will be available at Sauk Prairie Healthcare to help you explore your options:
WHEN: Thursday, March 21
TIME: 9am-12pm
WHERE: Sauk Prairie Hospital, Prairie du Sac [Enter hospital lobby, follow signs]
WHAT TO BRING: Medicare Card, Medication List, and Medicare.Gov username and password if you have it.
If you are unable to attend one of these sessions, feel free to reach out to the Sauk County ADRC at 608-355-3289 for assistance or if living outside of Sauk County, contact your local ADRC office within your county of residence. Once this open enrollment period closes, you will not be able to change plans for the remainder of the year.
