Financial Assistance Plain Language Summary

In keeping with its mission, Sauk Prairie Healthcare is committed to providing extraordinary healthcare from the heart – one person at a time. As part of this commitment, Sauk Prairie Healthcare recognizes the financial needs of patients and families within the community we serve. In that regard, Sauk Prairie Healthcare may provide medically necessary healthcare services at a discounted rate to those who qualify for financial assistance.

To manage its resources and responsibilities and to provide assistance to the greatest number of patients in need, the Board of Directors has established these guidelines for providing Financial Assistance. Sauk Prairie's complete Financial Assistance Plain Language Summary can be found here.

Eligibility

In order to be eligible for financial assistance for medically necessary health care services, the patient must qualify for assistance under Sauk Prairie Healthcare’s Community Care & Financial Assistance Policy. Federal Poverty Guidelines are used when determining whether a person or family is eligible. For a list of poverty levels, click here.

The medical necessity of treatment will be based upon clinical judgment of the healthcare provider without reference to the financial status of the patient. All patients will be treated for emergency medical conditions regardless of their ability to pay or to qualify for financial assistance.

Financial Assistance Application

Patients wising to apply for financial assistance must submit an application and the supporting documentation to the Business Services Department. Patient Financial Specialists (located at 260 26th St Prairie du Sac, WI 53578) are available at the hospital campus to assist patients in completing the applications. These documents are provided free of charge.
