There are many places in our community where you can receive the COVID19 vaccination, including at select Sauk Prairie Healthcare facilities. Remember, you will not be charged for the COVID19 vaccine. 

Appointments for first or second doses of Pfizer COVID19 vaccine for anyone age 12 or older can be scheduled at many Sauk Prairie Healthcare locations.

Pfizer Vaccine for Ages 12-17

Due to minor status of those ages 12-17, we HIGHLY RECOMMEND, and in some cases require, that a parent/guardian accompany the minor to the vaccination. If you are unable to accompany your minor to this appointment, please contact your clinic to discuss options. Not a Patient?

Community members may call Sauk Prairie Hospital at 608-643-3311 to request a same-day COVID19 vaccination appointment Monday through Friday, 8:00 am to 5:00 pm. Check in for the scheduled appointment at the hospital’s main entrance, 260 26th Street, Prairie du Sac, WI. If you have insurance, you will be asked to present your insurance card. Expect the visit to last 30-40 minutes.  

To find other locations where you and your family can receive the vaccination, including outside our community, visit Vaccines.gov. You may need to schedule an appointment, but many locations accept walk-in visitors.
