Healthy Life grants available for area projects

Healthy Life grants available for area projects

A total of $20,000 will be available from Sauk Prairie Healthcare Foundation for area projects through their Healthy Life Grants program. The money from these grants will help fund projects to better the health and wellness of communities in our area.

Applications for the grants are due November 10, with a total of $2,500 available for any one project benefitting community health after January 1, 2018. Sauk Prairie Healthcare Foundation intends to send grant notification letters to recipients by December 31, 2017.

Those eligible to apply include medical practitioners excluding Sauk Prairie Healthcare, educators, schools and school districts, community employers, civic organizations and service clubs, government agencies, libraries, museums, historical societies and ad hoc groups.

Programs eligible for the grants should fall into the broad definitions of health, community betterment, education, disease screening, fitness and wellness assessment or research. The programs’ services must be delivered to residents within Sauk Prairie Healthcare’s service area, including Prairie du Sac, Sauk City, Black Earth, Mazomanie, Lodi, Plain, Spring Green and other neighboring towns.

Consideration will be given to programs of a finite scope and duration and to startup projects that display uniqueness, creativity and impact, and based on the number of people the project serves.

Questions can be directed to Patrice Luer, Executive Director of the Sauk Prairie Healthcare Foundation at 608-643-7226 or at foundation@saukprairiehealthcare.org .  For more information and to apply for a grant, visit saukprairiehealthcare.org/grants
